With time, there is a loss of volume in the face.  Men and women that used to have youthful and high cheekbones often notice that this area of their face has flattened or drooped.  This is caused by both aging and gravity, which cause the soft tissues of the cheek to move downward and lose volume over time.  Before good injectable fillers were available on the market, cheek implants were often inserted to form full high cheekbones.  However, cheek implants are fraught with problems, like poor positioning, movement, infection, and numbness of the cheek.  Also, cheek implants need to be inserted in the operating room under anesthesia.  Rather than use cheek implants, I use either injectable fillers or a person’s own fat to enhance the cheek area.

Juvederm Voluma for Cheek Augmentation

I use Juvederm Voluma or Radiesse to augment the cheekbone area.  This is done in the office and the entire visit takes about 15 minutes.  The skin can be numbed with a topical anesthetic, although this is not needed in the vast majority of patients.  The risk of bruising is low since the injection is deep.  Juvederm Voluma is FDA-approved specifically for enhancement and filling of the cheek area.  Juvederm Voluma lasts up to 2 years, which is longer than any non-permanent filler on the market.  It absorbs and holds on to water to create fuller, more youthful cheeks and cheekbone areas.

Fat Transfer to Cheeks

If the patient desires an even longer lasting effect, fat can be harvested from the abdomen, hips, or thighs.  The fat is then processed so that it can be re-injected into the cheekbone area.  This procedure is called fat transfer or fat grafting to the cheek.  It is a slightly longer procedure, but can also be done in the office procedure room using sterile technique.  The benefit of fat injections over Juvederm or Radiesse is that the fat is all natural and about 60-70% of the fat remains after the first few months and is then permanent.  Fat transfer to the cheeks is often performed during a facelift.
