Breast Conservation with Lumpectomy or Partial Mastectomy
With improved breast cancer screening and imaging, the majority of breast cancers in women are diagnosed at an early stage. This means that many women can be treated with breast conservation, where a lumpectomy or partial mastectomy is performed, sparing the rest of the breast tissue. When the defect is very small compared to the size of the breast, sometimes nothing needs to be done in terms of reconstruction. However, after lumpectomy, the majority of women need radiation therapy as well.

What Can Happen WITHOUT Oncoplastic Reconstruction?
Radiation causes internal scarring of the skin and underlying tissue. If the lumpectomy is in the upper part of the breast, radiation can cause a scarred indentation where the tumor was taken out. This is often visible when wearing a shirt or dress. If the lumpectomy was in the lower part of the breast, radiation often causes the nipple to be pulled down towards the lumpectomy defect. It remains scarred in this position and this is very difficult to correct after radiation.

What is Oncoplastic Breast Reconstruction?
A new technique called oncoplastic surgery has emerged in the past few years which allows reconstruction after partial mastectomy. Oncoplastic surgery uses incisions and techniques from breast lift and breast reduction surgery to fill in the defects left after lumpectomy or partial mastectomy.
Benefits of Oncoplastic Lumpectomy Reconstruction
Oncoplastic incisions are less visible compared to the traditional incisions used by general surgeons. As long as a woman's breasts aren't too small, there is usually enough tissue to internally move around to fill in the defect. This prevents the concavity, indentation, or pulling of the nipple and areola after radiation.
Breast Lift or Reduction with Lumpectomy
The other benefit of oncoplastic surgery in women with large or sagging breasts is that the breast is lifted or reduced in size. This is usually combined with a lift or reduction on the opposite breast during the same procedure to give nice symmetry between the breasts. The surgery on both breasts is covered by the insurance company.
